Field Trip Parent/Guardian Form

If you want to attend a field trip with your school child at Parkwood-Upjohn, OR volunteer in the school, you MUST have a signed Record Check Form on file at the office.  This form MUST be updated annually.  Please fill out this form before attending the field trip with the classroom.  Questions regarding the form can be directed to Taray Stratton at (269) 337-0720.
