Teacher Supply Request List
We asked our teachers what they needed to have a successful school year. Because we know the parents of Parkwood-Upjohn Elementary enjoy helping out, a list was compiled of frequently needed items. If you have resources and want to help, here are some items that will help out. Please bring supplies to the office or ask what items your teacher could use. THANK YOU!
Field Trip Parent/Guardian Form
If you want to attend a field trip with your school child at Parkwood-Upjohn, you MUST have a signed Record Check Form on file at the office. This form MUST be updated annually. Please fill out this form before attending the field trip with the classroom. Questions regarding the form can be directed to Sheri Ferrari at (269) 337-0720.
